COVID-19 Vaccine Survey for Faculty and Staff

Through our partnership with the Louisiana Department of Health (LDH) and other state agencies, the University of Louisiana System is working to provide COVID-19 vaccination to faculty and staff in accordance with LDH vaccine tier distribution. To help make vaccine distribution and administration as efficient and effective as possible, we have been asked to survey faculty and staff about their willingness and availability to receive vaccination in the coming weeks.

• Any interest expressed in receiving the vaccine will be non-binding.
• Your survey answers will not be used for any other purpose.
• Your survey answers will only be available to those individuals responsible for creating the protocol, scheduling and administering of the COVID-19 vaccine.
• This survey is not a condition of employment; you are responding voluntarily.
